Toothache or Gum Pain? How Dentists Narrow the Cause
Tooth pain and gum pain can overlap. The mouth is small, but the source of pain is not always obvious, and the painful spot is not always the true source.
What to notice before the appointment
Before the visit, try to notice where the pain starts, how long it has been there, and whether it is constant or comes and goes. Tell the dentist whether hot, cold, sweets, biting, chewing, or tapping makes it worse. Swelling, bleeding, fever, and a recent injury are also useful clues.
Clues that can point one way or another
These patterns are clues, not diagnoses. Biting pain, cold sensitivity, and pain when a tooth is tapped can suggest a tooth source, including a crack. Bleeding, swollen tissues, and sore gums can point more toward gum inflammation or gum disease. Early gum disease may not hurt much, so the absence of pain does not rule it out.
What the dentist checks in the chair
A dentist usually starts with a careful history and an exam. That may include looking closely at the gums, checking pocket depths around teeth, tapping a tooth, asking you to bite on something, and using pulp or vitality testing to see how the tooth responds. If a crack is suspected, the dentist may check the tooth from several angles because cracks can be hard to see.
When X-rays help
X-rays can add important information, but they are not a substitute for the exam. They are used when the symptoms and findings justify them, not on a fixed one-size-fits-all schedule. Imaging can help show decay, bone changes, an infection around a root, or other problems that are not easy to see with the naked eye.
Why the same pain can come from different places
Some tooth pain is referred pain, which means the pain is felt somewhere other than the true source. That is one reason dentists may need more than one test before they can narrow the cause with confidence.
When to seek prompt care
