What to bring

• A current list of medicines and supplements you take, if possible with doses and how often you take them. Include prescription drugs, over-the-counter medicines, vitamins, and herbal products.
• Your allergy list, especially allergies to medicines, latex, or other materials used in dental care.
• A short symptom timeline: when the pain or injury started, what happened, what makes it better or worse, and whether there is swelling, bleeding, fever, or trouble opening your mouth.
• Coverage or member-ID details if you have them, such as CDCP information, a Healthy Smiles Ontario card, or private dental insurance details.
• The tooth, fragment, crown, filling, denture piece, or broken restoration if something fractured or was knocked out.
• Any relevant dental appliance involved in the problem, such as a denture, retainer, or night guard, if it is broken or part of the issue.

For children and dependents

Bring the child’s or dependent’s health and dental coverage information if you have it, along with the same medicine, allergy, and symptom details. If you are not sure which program applies, bring the card or paperwork you do have and the office can help sort through the rest.

If you do not have everything

Do not wait just because you cannot find every document. Come in with what you have. A dental team can usually fill in missing details during the visit.
