If a tooth is close to a nerve, an implant site needs careful mapping, or a <a href="https://exceldental.ca/category/root-canal/">root canal</a> problem is not fully clear on standard images, a dentist or specialist may discuss a cone-beam computed tomography scan, often called a <a href="https://exceldental.ca/category/cbct-scan/">CBCT scan</a>.</p>
<p>CBCT is a 3D dental scan. It can show teeth, roots, bone, nerves, sinuses, and nearby anatomy in more detail than a standard 2D dental X-ray. The important question, however, is not whether 3D imaging is newer. It is whether the scan is likely to answer a specific clinical question that could change diagnosis, safety, or <a href="https://exceldental.ca/tag/treatment-planning/">treatment planning</a>.</p>
<p>That distinction matters. Current guidance from the American Dental Association and a 2026 evidence-based radiography recommendation support selecting dental imaging based on the patient’s history, exam findings, previous images, and the clinical question being asked. In other words, CBCT is a selective planning tool, not a routine replacement for regular dental X-rays.</p>
<h2>What a CBCT scan can show that 2D X-rays may miss</h2>
<p>Standard dental X-rays are still the right starting point for many situations. They are often very useful for checking decay, bone levels, infections, and other common concerns. But because they flatten a three-dimensional area into a two-dimensional picture, some details can overlap or be harder to judge clearly.</p>
<p>A CBCT scan creates a 3D view of a limited area of the mouth or jaws. Depending on the reason for the scan, that added detail may help a dentist or specialist assess:</p>
<ul>
<li>the height and width of bone</li>

<p>Most routine endodontic care begins with clinical examination and standard dental images. But a CBCT scan may be considered when 2D imaging is inconclusive or when the anatomy appears complex. <h2>When standard 2D X-rays are often enough</h2>
<p>Many dental visits do not call for 3D imaging. Standard X-rays are often enough for:</p>
<ul>
<li>many routine checkups and recall visits</li>
<li>basic cavity detection when imaging is indicated</li>
<li>many straightforward toothaches or infections</li>
<li>monitoring many common dental conditions</li>
<li>many uncomplicated treatment plans where 3D information is unlikely to change the next step</li>
</ul>
<p>If a dentist says a CBCT scan is not needed, that is not a sign of missing technology. It may simply mean that standard imaging already answers the clinical question well enough.</p>
<h2>How dentists try to minimize radiation</h2>
<p>Dental imaging should be justified, not automatic. The American Dental Association notes that radiographs should be prescribed only when necessary for diagnosis and treatment planning. For CBCT specifically, the 2026 patient-selection recommendations support using it when lower-exposure options will not provide the information needed.</p>
<p>In practice, that means dentists and specialists try to reduce unnecessary exposure by:</p>
<ul>
<li>starting with a history and clinical exam</li>
<li>reviewing any recent X-rays that already exist</li>
<li>ordering imaging only for a specific clinical reason</li>
<li>choosing the smallest field of view that answers the question</li>
<li>avoiding 3D scans when 2D images are enough</li>
</ul>
<p>Patients do not need to memorize imaging terminology to ask good questions. It is reasonable to ask what the scan is expected to show and whether it is likely to change the plan.</p>
<h2>Ontario rules patients should know</h2>
<p>Ontario adds an important patient-safety safeguard. According to the Royal College of Dental Surgeons of Ontario, dentists or specialists who prescribe, order, take, interpret, or report on CBCT scans must be CT-authorized by the <a href="https://www.rcdso.org/en-ca/standards-guidelines-resources/rcdso-news/articles/17784" rel="nofollow noopener" target="_blank">RCDSO</a>.</p>
<p>This is also why referrals matter. If a general dentist believes a CBCT scan may be helpful, that dentist can identify the possible need and refer the patient to a CT-authorized dentist to determine whether the scan is clinically justified. The referring dentist does not need CT authorization if they are not prescribing, ordering, taking, interpreting, or reporting on the scan.</p>
<p>The RCDSO also says referrals for CBCT acquisition should include enough clinical information to justify the scan. That includes relevant history, a provisional diagnosis or proposed treatment, and copies or reports of recent images from the area of interest when available. This helps reduce inappropriate or unnecessary imaging and supports safer treatment planning.</p>
<h2>Questions to ask before agreeing to a CBCT scan</h2>
<p>If a 3D scan is recommended, it is reasonable to ask:</p>
<ul>
<li>What question will this scan answer?</li>
<li>Could the result change the treatment plan or improve risk planning?</li>
<li>Do my existing X-rays already answer the question?</li>
<li>Is a smaller field of view possible?</li>
<li>Who will interpret the scan and explain the findings?</li>
</ul>
<p>These questions are not challenging the dentist. They are part of informed decision-making.</p>
<h2>The bottom line</h2>
<p>CBCT can be very useful in selected situations, especially when a complex implant, root canal, wisdom tooth, surgical, or periodontal decision depends on anatomy that standard images cannot show clearly enough. But it is not a routine replacement for regular dental X-rays, and it should not be ordered simply because it is available.</p>

<p>When patients ask whether a Fotona laser can be used for a root canal, they are usually asking about one specific part of treatment: using an Er:YAG laser to activate the cleaning fluid inside the root canal.</p>
<p>That is an important distinction. A laser does not replace a root canal. Standard root canal care still depends on a careful diagnosis, shaping the canals with instruments, using disinfecting solutions, sealing the canal system well, and following the tooth over time.</p>
<p>In plain language, a Fotona laser may be used as an added tool during the cleaning phase. It is not a shortcut, and it is not a guarantee of a better result in every case.</p>
<h2>How Er:YAG laser activated irrigation fits into a standard root canal</h2>
<p>A root canal is done to treat infection or inflammation inside a tooth. After the dentist or endodontist reaches the inside of the tooth, the canals are cleaned with instruments and rinsed with disinfecting solutions. Because root canals can have tiny branches and irregular spaces, clinicians have looked for ways to help those fluids move more effectively.</p>

<p>A 2024 review in <em>Photobiomodulation, Photomedicine, and Laser Surgery</em> found that erbium lasers have shown the ability to disrupt biofilm in root canal systems, which helps explain why clinicians are interested in them as an adjunct. At the same time, this type of evidence is still stronger for mechanism and laboratory rationale than for proving clear clinical superiority in every patient situation.</p>
<h2>What the best recent evidence says about short term pain after treatment</h2>
<p>The most patient-relevant question is often simple: will I be less sore afterward?</p>
<p>A 2026 systematic review and meta-analysis in the <em>Journal of Endodontics</em> compared laser-activated irrigation with ultrasonically activated irrigation for postoperative pain after <a href="https://exceldental.ca/tag/root-canal-treatment/">root canal treatment</a>. The overall finding was cautious but clinically meaningful: laser-activated irrigation was associated with lower short-term postoperative pain, especially in the first 24 to 48 hours, and the benefit appeared strongest with pulsed Er:YAG approaches such as PIPS or SWEEPS.</p>
<p>That does not mean every patient will notice a difference. It also does not mean the root canal will feel easy afterward. Some soreness can still happen after any well-done root canal because irritated tissues around the tooth may need time to settle.</p>
<p>Still, if a dentist recommends Er:YAG laser activation as part of root canal care, the best recent evidence supports a careful statement that it may improve comfort in the first day or two for some patients when compared with ultrasonic activation.</p>
<h2>What the evidence says about cleaning, bacteria, and healing limits</h2>
<p>This is where it is important to be balanced.</p>
<p>Patients sometimes hear that lasers clean better, kill more bacteria, or make treatment more successful. Those claims need context. A 2024 randomized clinical trial in patients with apical periodontitis compared SWEEPS with passive ultrasonic irrigation and found similar effects on bacterial load and inflammatory markers between the two methods. In other words, one well-designed clinical study did not show a clear advantage for SWEEPS on those measures in that patient group.</p>
<p>That does not mean lasers have no value. It means the evidence is mixed, and the strongest support right now is for possible short-term comfort benefits, not for broad claims of better long-term healing or consistently better bacterial reduction in every case.</p>
<p>The American Association of Endodontists has also taken a cautious position. Its statement on lasers in dentistry says laser use as an aid in disinfection is more promising than laser use for canal preparation, but it also notes that effects on the clinical outcomes of root canal therapy were not established. That is an important message for patients. Even when a laser is used, the foundations of success are still diagnosis, case selection, good cleaning chemistry, careful shaping, sealing, and restoration of the tooth.</p>
<p>It is also important not to generalize findings from Er:YAG irrigation techniques to all dental lasers. Different wavelengths, tips, settings, and treatment goals are not interchangeable.</p>

<p>In Ontario, technology does not replace professional judgment. That matters whenever a dentist recommends a device-assisted approach.</p>

<p>For patients and families in Hamilton, that matters because it means AI is not just a marketing term. It is being discussed as a practical, ethical, and clinical issue. The important point is that AI in dentistry is being treated as a support tool that must fit within professional standards, not as a replacement for clinical care.</p>
<h2>What AI in dentistry usually means in a dental office</h2>
<p>In plain language, AI in dentistry usually means software that helps analyze dental images such as X-rays. It may highlight areas that look suspicious for decay, bone changes, or other findings so the dentist can review them more carefully.</p>

The review concluded that results were promising, but that further studies are still needed before these models can be confidently integrated into everyday practice across real-world scenarios.</p>
<p>For patients, the practical takeaway is simple. AI may help your dentist review an image more consistently or notice an area worth a second look. That is useful, but it is not the same as proving better long-term health outcomes for every patient.</p>
<h2>What the newer evidence shows: modest improvement, especially fewer false positives in one 2025 trial</h2>

<p>The current evidence base is growing, but it has important limits. Many studies look at how well software performs on image interpretation tasks rather than whether patients do better over time. <p>Many patients ask about laser dentistry after seeing it mentioned online or in advertisements. Lasers are used in a number of <a href="https://exceldental.ca/tag/dental-procedures/">dental procedures</a>, and they can be helpful tools in certain situations. However, they are not automatically better than traditional methods, and they are not appropriate for every procedure.</p>
<p>As a Hamilton dentist who has practiced since 1986, my focus is always on diagnosis, prevention, and choosing the right tool for the right clinical situation. Technology matters, but careful treatment planning and informed decision-making matter more.</p>
<h2>What is laser dentistry</h2>
<p>A dental laser produces a focused beam of light energy. Depending on the type of laser and the settings used, that light energy can cut or reshape soft tissue, remove small amounts of hard tissue such as enamel or dentin, or help control bacteria in certain situations.</p>
<p>In general practice, there are two broad categories:</p>
